my wife holder of PMJJBY from 01.06.2015 onwards. Annual premium also updated. our issue is on the certificate DOB mentioed wrongly. approached SBT at where applied and received, but informed bank cant do anything no option for any correction.hence for the same to which office/whom we may contact for recorrect. With due regards, I am to inform you about sudden demise of my husband Sh. Surinder Singh S/o Sh. Jagat Ram who was account holder with your Bank account No. 0631000300084687 at Dhameta branch, Teh. Fatehpur, Distt. Kangra, H.P-176025. My husband was also holder of insurance policy under Pradhan Mangtri Bima Yojna. He was expired on 28.9.2017 in a bike accident in Pathankot-Nurpur (at bharwal) and my son sustained serious injuries almost Rs. 3 lac has been spent on his treatment so far. About three months ago we had applied for the insurance claim with all requisite documents at the respective branch but so far no amount has been credited to my account. Therefore, your goodself is very respectfully requested to credit the insurance amount to my account for subsistence of family.

My wife is covered under Prahran Mantri jeevan yojna .a sum of Rs 330.00 have been deducted for Bank account automatically in May 2017,She has expired on 6/2/2018.With in how many days we have to submit the claim for Rs 2 lakh.Her treatment was free.We have not yet received the death certificate so far,and we have not been given any policy.Where to apply and within how many days of death.

Yah policy hamne bank through open ki hain ( shamrao vithal co-op bank) parantu hame policy ke document nahi mile hai.
