With due respect, I would like to inform you that, my father Nikhil Ramjan Biswas was died at NBMCH on 11th November 2021. I had applied online from your portal for Death Certificate vide acknowledgement no. : ACK/D/2022/050484. But till date during tracking application, it is showing only "Application Submitted" from last about 25 days. No further proceedings has shown.
In such context, please do the need full from your end and let me inform the actual reason of delay. Hope you will realize the urgency and do accordingly. Your kind cooperation will highly obliged.
