    appointment for dental procedure by devender:

    please refer to my uhid no. 398918
    This is reference to my visit in dental department on 3.4.2023. The doctor advised me to take appointment on phone after getting necessary permission from cghs and sent the photo of permission granted by cghs for taking appointment for further treatment.
    I have got permission on 5.4.2023 and send the photos on whatsapp no. 89294 68900 at about 9.20 am on 5.4.2023.
    I also tried to contact on the abovesaid no. regarding appointment for date and time but couldnot succeed.
    Therefore it is requested to kindly do the needful and if possible to give appointment on 7.4.2023 or 8.4.2023 (any time) or after 10.4.2023 between 10.00 am - 11.00 a.m.
