Respected sir/mam I admitted my patient Yagya Dutt Pathak in your Rungta hospital malviya nagar jaipur under RGHS On date 15/1/2024 under Dr.Deepak Garg sir Neuro surgeon in ICU later on date 17/1 he was shifted out to 3 sharing room no. 7 bed no.1 discharged on 18/1/24 but 1 NCCT BRAIN REPORT OF SMS HOSPITAL JAIPUR (Original report taken from me by junior doctor on preparation for discharge summary when I ask him to return he told that Dr.Deepak will see it but later he didn't return that and blood investigation reports and X ray done in Rungta hospital that report file also not given us with discharge summary so kindly look into the matter and my humble request to administration department that kindly provide all original reports of Rungta hospital and NCCT BRAIN ORIGINAL REPORT OF SMS HOSPITAL (14/1/24) TO KAPIL SHARMA NICU NURSING STAFF MY RELATIVE,BUT DO ALL THIS BEFORE SENDING THE FILE TO MRD DEPARTMENT.PLEASE KINDLY DO FOR NEEDFUL THANKS A LOT.
