    Sir,
    I have applied for new LPG connection via online mode on 05.05.2015 as detailed below.

    WL. No :700000032815714
    But I failed to get online generated filled KYC form which will be submitted to you along with affidavit and other documents for further proceedings at your end.Please release KYC form immediately with a reply to my query.
