I have received the cylinder on 31/07/2015 and payment made to the delivery boy accordingly, but till date subsidy amount is not being credited in my a/c

my customer No 37500000051046054 (old 09331). My name is DR.R.M.KEOT (Dr.RANA MUKUT KEOT). but it is spelled and printed at the cash memo/ bill as DR.R.M.KOCH.It is not corrected even after request given to you or after correction in KYC form. Now I have given up the subsidy on july 2015, and the email i received from the indane also spelled my name as DR.R.M.KOCH. This is unfortunate and I request you kindly to correct the name as DR.R.M.KEOT
