Dear Madam,
My Nephew Biplob Nandy , S/O Lt. Jagadish Nandy an Ex ASI of West Bengal Police, age 45 yrs, unmarried reside permanently at Chowhati A Block G.s.f.p Primary School , Chowhati Rajpur Sonapur Area since 1995 . My Elder Sister Shikha Rani Nandy, age 70 years a widow family Pensioner of his Husband Lt Jagadish Chandra Nandy.
My Nephew was very strong and healthy in his teen age. But after crossing his 20 years of age he suffers in unknown fever and he treated in many good Hospital like PG Hospital, Kolkata , Vellore Chennai etc . But day by day his health was tremendously ditiriote by Rheumatoid Arthritis and finally his Backbone totally Stiff from lumber to below head with damage both the veins of waist/loin and also both the legs was damage for walking as invalid in move ment of body functions . He can't sit at all. He can stand with his abnormal body transplant movement. Now he is totally physically helpless in his life with this medication. As my sister was a family widow pensioner. If a Disability Certificate can issued from Government Medical Hospital with their residential District Jurisdiction then his name will attach with his mother's pension records. It will help him for getting disability pension after death of his mother.
His Medical treatment documents are attached with this letter also attached three videos of his current physical movement for your consideration please.
I prey to you kindly arrange to send the concerned person either from Sonapur- Rajpur Municipality side or District Hospital Baruipur or Health and Family Welfare Department, Government of West Bengal.
